COVID-19 VACCINATION REQUIREMENT FOR STUDENTS


The Nevada State Board of Health enacted an emergency regulation requiring all students enrolling in Nevada System of Higher Education (NSHE) institutions to receive a COVID-19 vaccination.

All students must provide documentation of having received a completed COVID-19 vaccination series before enrolling for in-person courses.  This means students must receive both doses of the Moderna or Pfizer vaccine, or the single dose of the Johnson & Johnson vaccine, or vaccinations approved by the World Health Organization (WHO) for international students.

All students will not be able to register for courses until documentation of a completed COVID-19 vaccination series or a medical or religious exemption has been approved and on file.

VACCINATION REQUIREMENTS AND HOW TO SUBMIT DOCUMENTATION


All students who want to enroll in classes will need to complete a Student COVID-19 Vaccination Form and upload proof of vaccination series or a completed medical or religious exemption form.

  • You will need to know your NSHE ID number when completing the Student COVID-19 Vaccination Form.
  • The form requires your primary name as listed on your MyCSN student account and not your preferred name.
  • The front and back of your photo ID will be required to be uploaded with your documentation.
  • Submissions will be verified for completeness by the Office of the Registrar during college business hours in the order they are received
  • If you are applying for a medical or religious exemption, you will receive an email at your CSN student email account within 10 college business days letting you know the status of the immunization documents that were submitted.
  • Colleges Medical Immunization Exemption Form and Colleges Religious Immunization Exemption Form can be found on the NSHE website at https://nshe.nevada.edu/info-center/nshe-forms/
  • Once processed and approved, enrollment holds on your MyCSN student account will be automatically released and you will be able to register for Spring 2022 courses on your enrollment appointment date and time and thereafter.
  • Acceptable versions of the COVID-19 vaccination are:
    • One-Dose Johnson & Johnson
    • Two-Dose Pfizer
    • Two-Dose Moderna
    • Vaccinations approved by the World Health Organization (WHO) for international students

Note: You must have validated your student account at least 24 hours prior to submitting your COVID-19 vaccination form and supporting documents.

Before you submit, please have the following ready:

  • Proof of vaccination or completed exemption form
  • Valid government-issued photo identification
  • Minors submitting a religious exception form must have a parent signature and birth certificate/or other proof of legal custody.
  • Make sure your pop-up blocker is disabled in your web browser

FAQs

Is CSN open?

Yes, all CSN locations are open. Students can make in-person appointments with specific departments at this time. Please search the CSN website for contact information for the specific department from which you need support.  If you are unsure of who to contact, reach out to the CSN Call Center at (702) 651-5555 for assistance.

Are face coverings required on campus?

Yes, face coverings are required for all unvaccinated and vaccinated NSHE students, employees and members of the public while inside any building or structure on any NSHE campus or property. Unvaccinated students who cannot wear a mask due to medical reasons are to contact the Disability Resource Center. Face shields cannot be worn in place of a mask, but may be worn in addition to a mask.

I have to teach or take a class vitually, but I don’t want to share my personal space with the class. What can I do?

There are several CSN-branded virtual backgrounds that can be used by students, faculty and staff. These backgrounds are intended to encourage everyone to use their cameras during virtual meetings (which increases engagement) without the fear of displaying their personal spaces behind them.

Is my specific class offered online?

CSN is offering a variety of classes in online, hybrid and in-person formats. Please check the class schedule, then login to MyCSN to complete the registration process.

Can I meet with an advisor?

Yes.  Please contact Academic Advising to schedule an appointment.

I need to take a placement test. How do I schedule that?

Contact the Testing Center for information on how to take a placement test.

Can I register for online classes only?

Yes, you will be able to build a schedule of online classes only, as many of our classes will be available in this format.

What does it mean that I’m registering for a “Hybrid" class?

A hybrid class will incorporate both online instruction and in-person instruction. Before you add your class to your shopping cart, read the class notes.

What is CSN doing to make sure students are safe when on campus?

All students, faculty, staff and visitors must comply with face covering requirements.  Keeping our campus safe can best be accomplished by individuals staying home when sick, completing a daily health assessment, reporting of a COVID-19 exposure, COVID-19 symptoms, close contact with a positive case, or a pending case through wellness@csn.edu.

What’s the difference between an online, in-person, and hybrid course?

Our Guide to Understanding the Class Schedule explains the difference.  When you register for classes, make sure to read the “Class Notes” section where you add the classes to your shopping cart.  This will tell you if you need to stick to a scheduled class time or if you can access the class at your own pace.

I plan to be 100% online for spring 2022, do I have to complete the Student Vaccine Verification process and upload my answers?

Yes, CSN requires ALL students to complete the Student Vaccine Verification process regardless of instruction modality.  If you are not fully vaccinated and plan to enroll in only online/remote or distance education classes, you will be able to complete the verification process choosing the online enrollment exemption. However, be aware that if at any time you register for a course that involves any in-person interaction, you must submit proof of vaccination or an exemption based on medical condition/religious belief, or the course will be cancelled.
